Use this form to register teens/preteens, preschoolers (younger than grade 1),
and friends of St. Matthias students,
who are not registered for CCD or as a CCD Teen Helper
or at St. Ann Preschool.

  **Grade 6 and above CCD students are dismissed to their parents. If a parent has not arrived, student should return to the event area/church and wait with his/her Parish Youth Leader/volunteer/staff.
